Strokes of the posterior circulation typically present with different symptoms than those of the anterior cerebral vascular territories. The symptoms are often harder to identify and do not correspond to the typical stroke signs, and they are not represented in most stroke scales. Due to the poorer perception of symptoms by affected individuals themselves or by observing laypersons, there is a delayed alerting. Due to a poorer perception in the prehospital emergency care, there is insufficient recognition and assignment to the necessary care pathways.
Strokes of the posterior circulation are often recognized too late and therefore receive less frequently a timely and effective therapy. Stroke scores commonly used so far for a reliably prehospital diagnosis insufficiently cover disorders of the posterior cerebral circulation.
The Gugging Swallowing Screen (GUSS) assesses the risk of aspiration and the severity of post-stroke dysphagia, and gives dietary recommendations to prevent aspiration pneumonia.
